Police have determined the cause of death for a Vermont prisoner who was found dead at Northwest State Correctional Facility earlier this summer.

Investigators said 37-year-old Shawn Gardner, of Newport, was found with an unknown substance in the shower area prior to his becoming unresponsive at the facility on the evening of July 27.

The report said that Gardner had a fatal mixture of fentanyl, xylazine and cocaine in his system. His death has been ruled an accident.

Officials said Gardner was a federal inmate housed in a segregated unit of the facility. He was sent to the Northern State Correctional Facility on July 25 following his arrest but had been transferred to the St. Albans prison prior to a hearing in federal court on Wednesday.
